Hospital to Home

Our Hospital-to-Home Project succeeded because Health Research for Action worked with many community partners to assess the services needed by older adults transitioning from hospital to home in four Bay Area counties. We are continuing our work with several partners to develop and evaluate model programs and improve data collection tools.

• We collaborated with 30 community organizations to conduct focus groups and interviews with caregivers, providers, and policymakers, and case studies with families and isolated seniors.
• A community advisory board consulted with HRA on all aspects of the project. Many board members continue to be involved as we develop interventions based on our research.
• The American Society on Aging facilitated the involvement of local health professionals.
• The Center for Advanced Study of Aging Services, a nationally recognized center on aging at UC Berkeley, contributed to the analysis.
